Media Arts Parent Permission Form - 2023

Dear Parent/Carer,   

On behalf of the Creative Industries faculty I would like to welcome you and your child to the exciting world of Media. This semester, your child will be participating in the subject of Media, which includes the production of both film and photography elements and as such requires additional permission to undertake some activities.  Therefore, this letter provides information and details as to specific equipment and activities your child may undertake as part of this program of instruction. 

The Media courses delivered at Ripley Valley State Secondary College require the students to be instructed on the use of a variety of materials and equipment in the production of their assessable work. Students receive comprehensive instructional training in the safe use of all Media equipment and processes and are under close supervision until competent. Students competency allows them to obtain a Camera Licence before they are able to use the equipment, it is during this part of the course students are taught about Safe Handling – including set-up and pack-up, Camera and Cinematography procedures and equipment and lesson expectations.

Due to the nature of this course, it is required students complete the Camera Licence before being able to handle Media equipment, including cameras in any way. This is undertaken during class time, guided by their Media teacher.
Students understand the correct procedure when handling cameras and any inappropriate behaviour and misuse of equipment in unacceptable in the completion of this.

We strongly encourage students to participate in the use of this equipment and through individualised, competency-based training we teach students how to manage risks safely. Conversely students who do not demonstrate a mature attitude and follow safe operating procedures will be restricted in their use of the equipment.

Before your child can use the equipment involved in Media production, both you and your child are required to sign this electronic consent form.

PARENTAL CONSENT FORM 

Your child is participating in Media Arts offered at Ripley Valley State Secondary College for 2023. This course require the students to be instructed on the use of a variety of materials and equipment in the production of their work.

As a part of this consent you acknowledge that you accept liability for all costs incurred for the damage, breakage or loss of any Media equipment and undertake to reimburse the State of Queensland (via the Department of Education and Training) the full amount of any costs incurred on my child’s behalf. Below, are potential costs that could be invoiced for equipment used in this course:

While we do not force students to use the equipment if they do not feel confident and safe, it is an essential element of these courses that prepares them with skills and experiences for future work and exposes them to a wide range of technical equipment.

 

In addition to the State School Consent Form that was filled out upon enrolment at Ripley Valley State Secondary College, additional consents are required due to the nature of the subject with specific regard to media permissions and distribution of student work. Specific consent is required for the purpose of subject content and assessment, this includes photography and film content where the student may:

Have created Film or Photographic content

Featured as a subject in Film or Photographic content

As a part of this consent you acknowledge that Permission to Publish and Distribution of creative content is required as a part of this course.

Where students have created and participated in content created in Media, this may be used by Ripley Valley State Secondary College for Promotional purposes within the school and for external promotional uses.

Signing this form you have understood the Media Arts equipment expectations and safety rules and agree to the enforcement of them for all students safety. I am aware that if my child breaches the rules, my child could be excluded from all Media workshops due to the danger they pose to themselves and others.

Note: This Agreement Form will be retained on file at the School till the end of 2023.
