DAT Parent Permission Form - 2023

Dear Parent/Carer,   

On behalf of the Creative Industries faculty I would like to welcome you and your child to the exciting world of Design and Technology. This semester, your child will be participating in the subject Design and Technology, which includes Food Technology, Design Technology, Engineering and Manufacturing and as such requires additional permission to undertake some activities. At Ripley Valley State Secondary College we take the safety and wellbeing of our students seriously.  Therefore, this letter provides information and details as to specific equipment and activities your child may undertake as part of this program of instruction. 

Please take the time to read this important information which includes:

  • Information Communication and Technology requirements

Before your child can use the equipment involved in Design and Technology, both you and your child are required to sign this electronic consent form.

PARENTAL CONSENT FORM 

Your child is participating in Design and Technology offered at Ripley Valley State Secondary College for 2023. This course require the students to be instructed on the use of a variety of materials and equipment in the production of their work. A number of typical resources and processes have been designated by Education Queensland to have a potential medium, high or extreme risk of injury associated with their use.

As a requirement of our risk assessment process, it is recommended for risks designated MEDIUM, HIGH, and required for risks designated EXTREME, that the school obtain additional parental consent, including relevant medical information for students, before permitting them to use the Technology resources indicated below.

High Risk: Metal & Wood Lathes, Bandsaw, CNC Routers, Laser Cutter, Spot welder, Drill press, Spray painting plant, Pedestal Grinder and similar.
Extreme Risk: Thicknesser, Spindle Moulder, Surface Planer, Compound Mitre (Drop) Saw, Table Saw, Radial Arm Saw, Metal Cutting Bandsaw, Metal Cut-off Saw, Heavy sheet metal folder, Heavy guillotine, Angle Grinder and similar.

Possible Exposure to: Toxic Timbers & Wood Dusts including MDF & Formaldehyde, Plastics, Spray Painting Lacquers, Thinners & Solvents, Fiberglass & Resins, Oils, Compressed Air & Pneumatic Tools. Students may choose not to use equipment they do not feel confident with, similarly, students who do not demonstrate appropriate maturity or fail to complete safe operational training will be restricted in their use.

Year 7 & 8 – Note: Junior practical workshop curriculum activities are designed to a modified structure, thus minimising exposure to all medium, high and extreme risk plant & machinery. Students are, however, still likely to be exposed to some atmospheric wood dusts, lacquers, solvents and various plastics materials.

URGENT: Medical conditions including allergies must be kept current and on file through the school administration. Should you need to update your child’s medical details, please contact the school office on 3819 5333.

If you consent to your child’s participation in their selected Technology subject, utilising the resources as identified, please complete the consent details on the form below.

Signing this form you have understood the Technology expectations and safety rules and agree to the enforcement of them for all students safety. I am aware that if my child breaches the rules, my child could be excluded from all Technology workshops due to the danger they pose to themselves and others.

Note: This Agreement Form will be retained on file at the School till the end of 2023.
