RVSSC AARA Request Form

Access Arrangements and Reasonable Adjustments (AARA) are provided to assist students who have a disability, impairment, medical condition, or other circumstance, that may be a barrier to their ability to demonstrate their learning, knowledge or skill in assessment. This form is for students in Year 10-12 who are requesting additional support or adjustments for timed assessment items. 

This form is to be completed by the student and parent/carer and is automatically submitted upon completion to the RVSSC AARA review team for approval. Please complete this form for the following conditions:

  • Long-term and chronic conditions
  • Mental health conditions, such as anxiety and depression
  • Short-term conditions and temporary injuries

If you are wishing to request an extension for a temporary illness or misadventure, please complete the AARA (illness and misadventure) request form.

IMPORTANT NOTE

When submitting an AARA or Illness and Misadventure Request, it is important that this QCAA Confidential Medical Report form is completed by a medical professional and attached to the RVSSC AARA Request form.

If this information is not provided or completed correctly then an AARA may not be approved due to lack of supporting information.
